Building a Medical Home for Children

The purpose of this module is to provide information and insight regarding the medical home and how it benefits children. It is designed as a guide for users to better understand the medical home and how to build one. Particular focus will be placed on how health information technology (IT) can facilitate the creation and operation of medical homes. The questions below have been formulated to specifically address key topics related to the role of health IT in a medical home:

How can a medical home support transitions in care for children and how can health IT assist with these transitions?
