The Provider Relief Bureau (PRB) proactively posts information regarding Provider Relief Fund (PRF) payments on its website, including a list of all providers that have received and retained PRF payments and their total amount retained across all PRF distributions. The publicly available lists can be found on the PRF Data page and on the CDC's data page regarding the PRF.

In order to receive PRF records related to a specific provider, PRB requests that you provide the following information:
- Provider/Organization Name
- The Tax Identification Number (TIN) or the Employer Identification Number (EIN) of the entity you wish to search. TINs or EINs may be located in the National Corporation Directory or on the secretary of state website for the state in which the entity is located (e.g., Maryland Secretary of State Corporation and Business Entity Search (secstates.com)).
- If you cannot locate a TIN or EIN, please provide the provider’s/organization’s address and the National Provider Identifier (NPI). NPIs may be located in the National Plan and Provider Enumeration System (NPPES) NPI Registry.
Without a TIN or EIN, we require the provider’s/organization’s full name, full address and NPI number to properly search for responsive records.
