May 340B drugs be used for Medicaid fee-for-service patients as part of a 340B contract pharmacy?

340B drugs may not be used for Medicaid FFS patients at a contract pharmacy, absent an arrangement between the contract pharmacy, covered entity, and state Medicaid agency to prevent duplicate discounts. Any such arrangement shall be reported to the HRSA Office of Pharmacy Affairs by the covered entity. Once HRSA reviews and approves the arrangement, HRSA posts contract pharmacies that use 340B drugs for Medicaid FFS patients on the 340B OPAIS.
