- Who are pregnant
- Who live in rural areas and other communities in need
- Who received a transplant
Our programs also support the health workforce, health systems, and facilities. They provide care for these communities.
We do this by awarding grants to:
- States and territories
- Tribes
- Educational and community groups
What’s a grant?
A grant is federal money for ideas and projects that benefit the public.
Who can get a grant?
We award grants to organizations. These could be local or state governments. Our grants help them solve critical needs in their community.
What needs are critical?
A few examples:
- Expanding the health workforce
- Increasing access to affordable and quality health care
- More options for telehealth
- HIV medical care, medications, and support services

Fiscal Year (FY) 2025 Quality Improvement Fund – Transitions in Care for Justice-Involved Populations (QIF-TJI)
Who can apply:
Your organization must be a Health Center Program award recipient with an active H80 grant award to apply. Health Center Program award recipients are organizations funded under Section 330(e), (g), (h), and/or (i) of the Public Health Service Act.
Fiscal Year 2024 Behavioral Health Service Expansion
Who can apply:
You can apply if your organization is a Health Center Program award recipient6 with an active H80 grant award.
We encourage health centers that do not currently provide behavioral health services to apply.
Native Hawaiian Health Care Improvement Act Program
Who can apply:
This is a limited competition program notice.
To be eligible for administrative funding under this NOFO, you must be Papa Ola Lokahi.
To be eligible for services funding under this NOFO, you must be a Native Hawaiian Health Care System (NHHCS) that has been recognized and certified by POL as qualified to provide comprehensive health promotion, disease prevention, and primary health services that are tailored to fit the needs of the Native Hawaiian communities. Recognized and certified NHHCSs and their respective island communities are listed in Table 1.You must provide proof of Papa Ola Lokahi’s recognition and certification in Attachment 13, or your application will be considered non-responsive and will not be considered for funding.

You must be a domestic public or private, nonprofit entity, as demonstrated through the submission of the Evidence of Non-profit/Public Center Status (Attachment 11), outlined in Section IV.2.vi.8 Faith-based and community-based organizations, Tribes, and tribal organizations are eligible to apply
