Who can apply: You must be a domestic public or private, nonprofit entity, as demonstrated through the submission of the Evidence of Non-profit/Public Center Status (Attachment 11), outlined in Section IV.2.vi.7 Faith-based and community-based organizations, Tribes, and tribal organizations are eligible to apply.

Who can apply: All 50 States, the District of Columbia, the Commonwealth of Puerto Rico, the U.S. Virgin Islands, Guam, American Samoa, the Commonwealth of the Northern Mariana Islands, the Republic of Palau, the Federated States of Micronesia, and the Republic of the Marshall Islands are eligible to apply for funding if they meet the following requirements: Eligible applicants are limited to RWHAP Part B states/territories that: • have reported to HRSA an existing ADAP waiting list, • have used the ADAP ERF to prevent, reduce, or eliminate an ADAP waiting list between January 2011 and July 2021, • need additional funding for a projected increase in treatment needs aligned with ending the HIV epidemic in the U.S., or • need additional funding for other unanticipated increases in the number of clients in the program due to new diagnosis, re-engagement in care, loss of income, and/or loss of health care coverage. States/territories that did not: 1. report a new ADAP waiting list to HRSA, or 2. use the ADAP ERF to prevent, reduce, or eliminate an ADAP waiting list between January 2011 and July 2021, or 3. that do not have a projected increase in treatment needs aligned with ending the HIV epidemic in the U.S., or other unanticipated increases in the number of clients in the program due to new diagnosis, re-engagement in care, loss of income, and/or loss of health care coverage, are not eligible to apply.

Individuals are not eligible to apply under this NOFO. Who can apply: Eligible applicants for the HPSWRTAC are: Health professions schools, academic health centers, state or local governments, Indian Tribes and tribal organizations, or other appropriate public or private nonprofit entities (or consortia of entities, including entities promoting multidisciplinary approaches). Entities providing health care, including health care providers associations and Federally Qualified Health Centers (FQHCs). Individuals are not eligible to apply under this NOFO. Applicants that are institutions of higher education, health professions schools, and academic health centers must be accredited by a nationally recognized accrediting body. These applicants must submit their accreditation documentation as Attachment 4. Failure to attach a copy of the required accreditation documentation as Attachment 4 will be considered non-responsive, and applications will not be considered for funding under this announcement. Applicants are required to maintain their accreditation throughout the period of performance and notify HRSA of any change in status. Eligible entities must be located in the 50 states, the District of Columbia, Guam, the Commonwealth of Puerto Rico, the Northern Mariana Islands, America Samoa, the U.S. Virgin Islands, the Federated States of Micronesia, the Republic of the Marshall Islands, or the Republic of Palau.

Who can apply: An eligible entity is a community-based ambulatory patient care center that: i. Operates an accredited primary care residency program. Specific examples of eligible outpatient settings include, but are not limited to: • Federally qualified health centers, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)]; • Community mental health centers, as defined in section 1861(ff)(3)(B) of the Social Security Act [42 U.S.C. 1395x(ff)(3)(B)]; • Rural health clinics, as defined in section 1861(aa)(2) of the Social Security Act [42 U.S.C. 1395x(aa)(2)]; • Health centers operated by the Indian Health service, an Indian tribe or tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act [25 U.S.C. 1603]); and • A community-based entity receiving funds under Title X of the PHS Act.

Who can apply: An eligible entity is a community-based ambulatory patient care center that: i. Will operate an accredited primary care residency program. Specific examples of eligible outpatient settings include, but are not limited to: ? Federally qualified health centers, as defined in section 1905(l)(2)(B) of the Social Security Act [42 U.S.C. 1396d(l)(2)(B)]; ? Community mental health centers, as defined in section 1861(ff)(3)(B) of the Social Security Act [42 U.S.C. 1395x(ff)(3)(B)]; ? Rural health clinics, as defined in section 1861(aa)(2) of the Social Security Act [42 U.S.C. 1395x(aa)(2)]; ? Health centers operated by the Indian Health Service, an Indian tribe or tribal organization, or an urban Indian organization (as defined in section 4 of the Indian Health Care Improvement Act [25 U.S.C. 1603]); and ? An entity receiving funds under Title X of the PHS Act. The list of entities above is not exhaustive, but does reflect the intent of the program to provide training in community-based settings such as those served by the institutions listed. OR ii. Has collaborated to form a community-based GME consortium that will operate an accredited primary care residency program. In order to satisfy accreditation, academic and administrative responsibilities, a community-based ambulatory patient care center may form a GME consortium with stakeholders (e.g., academic health centers, universities and/or medical schools) where the GME consortium will serve as the institutional sponsor of an accredited primary care residency program. The relationship between the community-based ambulatory patient care center and the consortium must be legally binding, and the agreement establishing the relationship must describe the roles and responsibilities of each entity. Within the consortium, the community-based ambulatory care center is expected to play an integral role in the academic, financial and administrative operations of the residency. THCPD payments must be used to support residency planning and development activities at the ambulatory training site.
